By clicking above and signing below I hereby agree to accept a position as a volunteer worker for the Colorado Animal Rescue (herein after referred to as C.A.R.E.) and in doing so, I agree to comply with all of the rules and regulations which may be established from time to time by the and I understand that failure to do so may result in my immediate termination as a volunteer.

I acknowledge that my services are provided strictly on a volunteer basis, without any pay or compensation of any kind, and without liability of any nature on behalf of C.A.R.E. All services will be performed at my own risk.

I recognize that in handling animals and performing other volunteer tasks there exists a risk of injury including physical harm caused by the animals. On behalf of myself, my heirs, person representatives, and executors, I hereby release, discharge, indemnify and hold harmless the Colorado Animal Rescue, Inc, it’s agents, servants, and employees from any and all claims, causes of action, or demands, or any nature or cause, including costs and attorney’s fees incurred by the Society in connection with the same, based on damages or injuries which may be incurred or sustained by me in any way connected with my services for the Colorado Animal Rescue, including but not limited to animal bites, accidents, or injuries.

Volunteer Release Form for Minors (17 years or younger)

By signing below, I, being the parent or legal guardian of the volunteer who is under the age of 18, hereby give my consent to allow my (son, daughter, ward) to perform volunteer services for the Colorado Animal Rescue (herein after referred to as the “C.A.R.E.”). I fully understand and acknowledge that his/her services are to be performed subject to all the rules and regulations or the Shelter, that violations thereof shall be cause for immediate dismissal of all services, and that all services performed by my (son, daughter, ward) are strictly voluntary, without pay or compensation or any sort and without liability of any nature of behalf of C.A.R.E. I further acknowledge that all services are performed at his/her own risk. I am also aware that any person under the age of 16 must be accompanied by a parent or legal guardian at all times while volunteering at C.A.R.E.

On behalf of myself, my (son, daughter, ward), my heirs, my personal representatives or administrators, I hereby release, discharge, indemnify and hold harmless C.A.R.E., it’s agents, servants, and employees from any and all claims, causes of action, or demands, or any nature or cause, including costs and attorney’s fees incurred by C.A.R.E. in connection with the same, based on damages or injuries which may be incurred or sustained by him/her in any way connected with his/her services for the Society, including but not limited to animal bites, accidents, or injuries.
